ABSTRACT

This paper critically examines the multifaceted challenges impacting rehabilitation services in India, with a particular focus on accessibility, availability, acceptance, and cost-effectiveness for individuals with disabilities, especially those with intellectual disabilities. It further investigates the alignment of India's rehabilitation strategies with global policy frameworks, such as WHO's Rehabilitation 2030, to identify opportunities for enhancing integrated, community-centric healthcare and rehabilitation models. This review synthesizes evidence from a diverse range of academic literature, policy documents, and organizational reports to construct a comprehensive understanding of the current landscape of rehabilitation services in India. Specifically, it delves into how Community-Based Rehabilitation and Community-Based Inclusive Development can transition support services from a medical model to a human rights-based perspective, aligning with international mandates like the Convention on the Rights of Persons with Disabilities.  The research reveals that rehabilitation facilities in India are predominantly concentrated in urban centers and private sectors, posing significant accessibility challenges for individuals residing in rural areas and those with limited financial resources. This geographical and economic disparity severely restricts access to necessary interventions, exacerbating the health inequities faced by vulnerable populations, including children with intellectual disabilities who encounter cognitive, structural, and financial barriers to healthcare.  The current findings underscore the urgent need for a paradigm shift in policy and resource allocation to ensure equitable access to rehabilitation services across all demographic and geographic segments of India, particularly for those in rural areas with higher rates of neediness. This disparity is particularly pronounced given that only a small fraction of individuals have access to hospitals or primary healthcare facilities, further burdening caregivers of children with neuro developmental disorders, especially in rural and economically disadvantaged settings. This study provides a novel, integrated analysis of the interplay between accessibility, availability, acceptance, and cost-effectiveness within India's rehabilitation landscape, emphasizing the critical need for a human rights-based approach to address pervasive disparities. It further highlights the importance of leveraging strategies such as Community-Based Rehabilitation and Community-Based Inclusive Development to enhance both the reach and efficacy of these services, especially in resource-constrained environments.
